Registration Form Section (1-6 All 6 sections need to be completed before an athlete is allowed to practice or compete in an NCCS athletic competition. )
Recertification Form ( Section 7 This re-certification form must be completed prior to playing each additional sport at NCCS within the same school year.)
Recertification Form ( Section 8 This re-certification form must be completed by a physician once the student athlete has completed treatment for medical injury or illness.
